*RESTRICTIONS:
Ages 7-17 tandem students need to have both parents present. If one parent isn’t present, a notarized consent form is needed. A passport or birth certificate with a state ID is required of the passenger that has a matching address of the parents. The agreements need to be signed by the passenger & parents.
